This Service is produced and maintained by Acer e-Enabling Service Business Inc. commissioned by the Taiwan Centers for Disease Control (hereinafter referred to as Taiwan CDC). 個人資料之蒐集
利用本服務需申請人提供個人資料時，本服務會依案件辦理需求請您提供身分證號、健保卡 號、戶號、護照資料、申請人信箱等個人最新、最真實之資料。
When you use this Service, we will ask for your latest information such as your name, identification number, national health insurance card number, household number, passport information, e-mail address, etc.
本服務所需之英文姓名及護照相關資料，係由外交部領事事務局所提供，若不同意提供，則無法使用本服務。
Your English name and passport information necessary for the use of this Service are provided by the Bureau of Consular Affairs, Ministry of Foreign Affairs. If you do not agree to provide such personal information, you will not be able to use this Service.
